Sleep Apnoea

Still tired, still snoring — and CPAP isn't working for you?

For some people, obstructive sleep apnoea is driven by the size and position of the jaws. It may be worth a surgical assessment if:

  • You have diagnosed sleep apnoea and struggle to tolerate CPAP
  • You have a small or set-back lower jaw
  • Your partner reports loud snoring, pauses or gasping
  • You're sleepy during the day despite a full night in bed

Jaw Surgery for Obstructive Sleep Apnoea in Singapore

Skeletal surgery such as maxillomandibular advancement (MMA) to enlarge the airway in selected patients with obstructive sleep apnoea.

How Jaw Position Affects Breathing

In obstructive sleep apnoea (OSA), the upper airway repeatedly narrows or collapses during sleep. In some people, a small or set-back jaw contributes by reducing the space behind the tongue and soft palate.

Maxillomandibular Advancement (MMA)

MMA moves both the upper and lower jaws forward, which pulls forward the tongue and soft tissues attached to them and enlarges the airway. It is one of the more effective surgical treatments for moderate to severe OSA in suitable patients.

Other skeletal procedures, such as genioglossus advancement, may be considered depending on where the airway narrows.

Working With Your Sleep Team

Surgery is usually considered after a sleep study and assessment by a sleep physician or ENT surgeon, particularly for patients who cannot tolerate CPAP. Dr. Loo teaches on local and international surgical courses on corrective jaw surgery and obstructive sleep apnoea.

Frequently Asked Questions

Is MMA a first-line treatment for sleep apnoea?

No. CPAP and oral appliances are usually tried first. MMA is considered for patients with a skeletal cause who cannot use, or do not benefit from, other treatments.

Will MMA change my appearance?

Advancing the jaws usually changes the profile of the lower face. Although the surgery is performed to open up the obstructed airway, there is often an added aesthetic benefit: a stronger-looking jaw and less laxity in the lower face. This is discussed and previewed during 3D planning.
